- Current State Certified or Registration Contractors License
- Current Occupational Business Tax Receipt
- Current Certificate of Liability Insurance
- Current Worker's Compensation Exemption or Certificate of Worker's Compensation Insurance
Permit Applications may be submitted via. fasmilie. Any submital over 5 pages will be charged per page copy fee accordingly. Any require documents, attachemtns, blueprints, and drawing that are larger than legal size paper should be submitted in non-electronic (physical)format in person, mail or by courier.
