1.    • Criminal Conviction Worksheet, when requested
   • Copy of respective Connecticut Journeyperson license and the date first issued
   • Voucher for payment of the non-refundable application fee by check and/or money order to the "Treasurer, State of Connecticut"
   • For military service experience and training related to this application, contact the CT Department of Labor at (860) 263-6128 to request a recommendation
   • When applying for equivalent experience and training, include all documentation that demonstrates experience and training for Limited Gasoline Tank, Piping, and Pump (P-9)
     • Copy of each out-of-state electrical license, currently active
     • Notarized/certified letter from each state indicating good standing with the issuing board and the date that the license was initially issued
     • Proof of at least one of those states' requirements that meet or exceed those of the trade
     • Proof of successfully completed registered Apprenticeship Program from the DOL, including on the job training (OJT) and related instruction
4. Schedule the Limited Gasoline Tank, Piping, and Pump (P-9) administered by PSI© in the desired location and time and pay the examination fee after receiving the DCP examination approval (the exam must be taken within one year from the date of approval):
   • Take and pass the Limited Gasoline Tank, Piping, and Pump (P-9) and the Business and Law exams
