How to Get the Underground Storage Tank Services Provider’s License in Oregon?

1. Register for a Construction Contractors Board Online Services Account with the Construction Contractors Board if needed
2. Complete the application form and send it to the given address, along with the required fee and the following:
   • List of all licensed supervisors employed by the service provider, license number and type(s)
   • Evidence that the company has prior experience or relevant knowledge if sending a new application
   • Underground storage tank projects completed in the past 24 months if sending a renewal application
3. Receive a license valid for twenty-four months and issued by the UST Department once the application is validated
