Connecticut Lead Planner Project Designer License

The Lead Planner Project Designer License allows you to design:

• Lead abatement
• Management activities

Must be employed by a Lead Consultant Contractor who holds a valid license from the Connecticut DPH.

How to Get the Lead Planner Project Designer License in Connecticut?

1. Visit the CT Department of Public Health (DPH) website, and read the eligibility criteria, prerequisites, and documentation requirements for obtaining a Lead Planner Project Designer License. Follow the process held in the Environmental Practitioner Licensure
2. Apply online, by visiting the State of Connecticut's eLicense website or fill out the notarized Lead Consultant Certification Application. MUST be typewritten. Handwritten applications will be returned
   • Recent, glossy color, passport-type photograph affixed to the application
   • Receipt for payment of the non-refundable application fee by check and/or money order to the "Treasurer, State of Connecticut"
   • Provide all the documents when requested. It may include the following:
     • A legible copy of your Lead Training Course certificates:
       • Initial 16 -hour training certificate from a CT-approved training provider
       • Legible copy of an initial Lead Abatement Supervisor certificate from a CT-approved training provider
       • Verification of Experience Form
     • Confirm by signing page 4 of the Lead Consultant Certification Application, in the presence of a notary, have not worked in Connecticut in the discipline for which you are applying during the time the credential was expired
     • Complete the Statement of Professional History (page 3 of the Lead Consultant Certification Application) and provide the following documents, if applicable
       • Notarized Proof of settlement of fine
       • If ever been found guilty or convicted as a result of an act that constitutes a felony under the laws of the state, provide a Notarized Certified Court Copy (with court seal affixed) of the original complaint, answer, judgment, settlement, and/or disposition of the case
       • If currently on parole or probation, provide a statement from the officer with the conditions of release
     • Applicants credentialed in another State or jurisdiction seeking reciprocity certification must provide the following:
       • Statement to be included with your application to document you are seeking reciprocity certification in CT
       • Copy of your current refresher training certificate
       • Copy of your current license from another State or jurisdiction
       • Copies of your license from another State or jurisdiction or other supporting documentation to show you have been licensed in the other State or jurisdiction for no less than four years
3. Mail the completed application, the appropriate required documentation for the license type, and the correspondent fee receipt to the DPH Environmental Licensing Office
4. Await for the professional staff of the DPH to review the application and make an eligibility determination. The review will be conducted, and the applicant will be notified in writing with regard to the determination
5. Upon approval of an application, the licensee will receive written verification of the license number and the effective date. Three part licensing documents will be forwarded to the licensee's address
