Specialty Contractor Registration

Please fill out and return the application along with copy of the following items and submit it to the City of Homestead:

Miami-Dade County Contractors

• Certificate of Competency
• Miami-Dade Municipal Contractor’s Tax Receipt (MC)
• Miami-Dade Local Business Tax Receipt (LBT)
• State of Florida Contractor License/Registration
• Liability and Workers Compensation Insurance
• Driver’s License (original or clear & legible color copy only)

State of Florida Contractor's

• State of Florida Contractor License
• Liability and Workers Compensation Insurance
• Driver’s License (original or clear & legible color copy only)
• Miami-Dade Local Business Tax Receipt (LBT) or County of Origin

Liability and Workman’s Comp Certificates of Insurance must be made out to the following Certificate Holder:
