In the City of Miami Shores, the following documents need to be submitted along with thecontractor registration form:

• County Licensed:
  Driver's License
  State Registration
  County Certificate of Competency
  County Business Occupational License
  City of Miami Shores Business Tax Receipt
  Liability Insurance (City of North Miami Beach as certificate holder)
  Workman's Compensation or exemption (City of North Miami Beach as certificate holder)

• State Contractors:
  State License
  Driver's License
  QB License (if applicable)
  Business Occupational License
  Liability Insurance (City of Miami Shores as certificate holder)
  Workman's Compensation or exemption (City of Miami Shores as certificate holder)
