Utah Elevator Mechanic Licenses

The Elevator Mechanic License is issued by the Utah Department of Commerce Division of Occupational and Professional Licensing. To obtain it, the applicant must fulfill some requirements, present a ProV© examination and send an application to the Department.

How to get a Utah Elevator Mechanic License in Utah?

  1. Acquire the license, check the certificates and experience requirements:
  2. Apply online in ProV©:
    1. Register for the Elevator Mechanic Examination.
    2. Schedule and pay for the test.
    3. If scheduling by phone, call: 877-228-3926
  3. Pass the examination; the tester should score 75% or higher.
  4. Complete the license application process.
    • If applying with a certificate of completion, provide a copy of one of the following:
      • The Canadian Elevator Industry Education Program.
      • The National Elevator Industry Education Program.
      • The National Association of Elevator Contractors Certified Elevator Technician Education Program.
      • Any other program that meets the requirements established by the Commission.
    • If applying by experience:
      • Submit the Affidavit of Work Experience (page 3)
        • Must have 6000 hours of qualifying training
      • Send copies of the W-2 forms
  5. Send the application to the Department:
    1. If applying online:
      1. Register or sign in
      2. Fill out the corresponding information
      3. Pay the application fee
    2. If applying by mail:
      1. Fill out the license application form
      2. Send it to:
        Division of Occupational and
        Professional Licensing

  6. Expect a response from the Department in a 4 to 6-week period.
