Contractor License Registration Form

Below is a list of items that is required to register any State Certified, Registered or County Specialty License with Polk County Building Division.

      • Copy of current Business Tax Receipt/Occupational License from a county or city.
      • General Liability Insurance Certificate, listing the Polk County Building Division as the certificate holder.
      • Workers Compensation Insurance Certificate, listing the Polk County Building Division as the certificate holder, or a current workers compensation exemption.
      • There is no registration or renewal fee for State Certified Contractors.

If you are a Registered or County Specialty Contractor, in addition to 1 thru 3 above, you are required to provide:

      • Registration Fee – See Page two of this document. If you are registering after June 1st during an even year the fee will be half of the amount shown. Your registration must be renewed every two years (odd years) in conjunction with your license renewal.
      • RA Surety bond in the amount of $5000.00 (See County forms for Surety Bond verification form).
      • RIf you are registering for the first time you will need a letter of reciprocity from your sponsoring municipality. The required score for the business law and trade examinations is a minimum of 75% or higher in order to work in Polk County; in addition you must be in good standing with your sponsoring municipality.

Note: You are responsible for ensuring that current documents are maintained throughout the registration period. The expiration of any of the above documents will render your registration inactive and will affect your ability to obtain permits.

Contractor Licensing Board

        • The Contractor Licensing Board reviews complaints against, and exercises disciplinary authority over licensed contractors. This authority includes license suspension or revocation for locally licensed contractors, but is limited to suspension of permit pulling privileges in Polk County for state certified contractors.
        • The Contractor Licensing Board meets the second Thursday of each month in the County Commission boardroom. Meetings begin at 5:30 p.m and conclude when all cases on the agenda have been heard.

Qualifications for Board Members / How to Apply

The Polk County Board of County Commissioners appoints nine members and two alternates to the Contractor Licensing Board. Members are appointed for three-year terms, and there are no term limits. Members must also be residents of the county for at least one year, and must represent at least:

      • Two building contractors
      • One electrical contractor
      • One H.A.R.V. contractor
      • One plumbing contractor
      • One architect or engineer licensed by the state
      • One swimming pool contractor
      • One roofing contractor
      • One associate with the gas association
      • Two alternates
