• Mechanical parts of the installation, alteration, repair, and maintenance of elevator equipment

How to Get the Elevator Limited Mechanic (LEM) Contractor’s License in Oregon?

1. Fill out the application form:

2. Complete a work history document and fill out the experience verification form:
   • Elevator Experience Verification Form
3. Sign an applicant’s affidavit
4. Mail or fax a sealed packet of all the previous documents to:

5. Expect the license issued by the Building Codes Division via mail
