• Contractors must be registered with the City of North Port.
• You have to provide a copy of your State or County license.
• Authorized Agent Form (for anyone doing permitting other than qualifier).
• A Copy of the qualifier’s driver’s license.
• A Copy of current Worker’s Compensation (or exemption).
