In the Town of Medley, the following documents need to be submitted along with thecontractor registration form:
- Florida State Certified License & Miami-Dade County Certificate Of Competency.
- Florida State Registration & Miami-Dade Municipal Local Business Tax Receipt.
- Certificates Of Worker’s Compensation & Liability Insurance (Original Copy)
Town Of Medley Is To Be Named As The Certificate Holder. - Florida Secretary Of State Current Certificate
- Photo Identification Of Qualifier (Driver’s License)
- Copy Of Business Tax Receipt
- Copy Of Miami Dade Municipal Contractor’s License (If Not State Certified)
