Please fill out and return the application along with copy of the following items and submit it to the City of Coconut Creek:

• Proof of Liability Insurance
• Qualifiers State Driver’s License
• Proof of Workers’ Compensation/Exemption
• Florida State Certification or Broward County Certification with Broward County Business Tax Receipt
